Identifying needs and connecting members: Our SDOH risk model pinpoints vulnerabilities and connects members to crucial services like food banks or housing assistance.
Breaking down barriers to care: Telehealth expands access for those facing transportation obstacles, ensuring they receive timely and essential healthcare.
Convening for collective impact: We act as catalysts in our communities, partnering with models like Wellness Trusts and our own Social Health Bridge to secure sustainable funding for SDOH initiatives.
2. Transportation and Health: A Tangible Link:
The link between transportation and health is undeniable. 3.6 million Americans miss medical appointments each year due to transportation barriers, according to the AHA. This often leads to higher costs and poorer health outcomes for our members.
3. Addressing Barriers to SDOH Initiatives:
Budgetary constraints and lack of reimbursement remain the biggest hurdles for payers and providers (55%). A study by George Mason University and Harvard Business School suggests that demonstrating ROI from SDOH investments could significantly increase provider buy-in.
